Your USAble Life Accident Plan purchased through a Florida Blue agent or the website includes a Wellness Benefit that pays a cash benefit for a covered wellness service/test to support your health and well-being. Not only do you get cash in hand, but you can unlock the potential for a healthier you.

The Wellness Benefit allows one covered person on your policy to receive a cash benefit for one covered wellness service/test each year. If you missed filing your 2024 claim, there’s still time, and it’s easy to file online. Things you will need

  • Your policy number (can be found on your email and letter received)
  • Wellness date of service
  • Dependent’s Wellness date of service (if claim is for a dependent benefit)
  • Name and contact information for health care provider(s)

Please note

You must provide all the necessary information in one session. All sessions are closed after 30 minutes of inactivity. If you are unable to complete your submission in this session, you will be able to start the process over, but the information you previously entered will be lost. Prior to submitting the Claim form, you may print a copy of the completed form for your records.

About your Wellness Benefit
